Application Forms

If you are resident Jamaica or the Bahamas you need to use an APS2E passport application form. This is not the same form as the one used by persons who reside in Ireland.

APS2E forms are available from:

• The Consulate of Ireland. You can contact us here. Please include your name and full postal address, and please state how many forms you require.

Please note that completed applications should be sent to the Embassy of Ireland in Canada and not to the Consulate of Ireland in Jamaica
